WAIVER AND RELEASE OF LIABILITY

I, the undersigned, acknowledge and agree to the following:

1. Assumption of Risk 
   I understand that participation in physical fitness activities, including circuit training and strength-based classes, involves inherent risks including, but not limited to: muscle strains, sprains, falls, equipment-related injuries, heart conditions, and other physical injuries or conditions. I voluntarily assume full responsibility for any risk of injury, illness, or death arising from my participation.

2. Medical Clearance 
   I affirm that I am in good physical condition and do not suffer from any known disability or condition that would prevent or limit my participation in this fitness program. I agree to consult with a physician prior to starting any new exercise program, including those provided by Project Phoenix.

3. Release of Liability
   In consideration of being permitted to participate in classes or use the facilities at Project Phoenix, I hereby release, waive, and discharge Project Phoenix, its owners, employees, contractors, trainers, and agents from any and all liability, claims, demands, or causes of action related to any loss, damage, or injury that may be sustained while on the premises or participating in any gym-related activity, whether caused by negligence or otherwise.

4. Cancellation Policy 
   I understand that Project Phoenix requires a 30-day written notice for the cancellation of any membership, program, or recurring billing. Cancellations must be submitted via email and will be effective 30 days from the date of notice.

5. Non-Solicitation and Voluntary Participation 
   I affirm that I am joining Project Phoenix of my own free will and have not been solicited, pressured, or coerced by any representative, member, or affiliate of the gym. I have chosen to train with Project Phoenix based on personal preference and without any prior communication, promotion, or inducement by the company. 

6. Photo & Video Release (Optional)
   I grant Project Phoenix permission to use photographs or videos taken of me during classes for promotional and marketing purposes. I understand that I will not be compensated for such use.

7. Rules & Conduct
   I agree to abide by all posted safety rules and gym policies. I understand that failure to follow these rules may result in suspension or termination of my gym access without refund.

8. Emergency Medical Care
   In the event of a medical emergency, I authorise Project Phoenix staff to obtain emergency medical treatment on my behalf, including calling emergency medical services.

PHYSICAL ACTIVITY READINESS QUESTIONNAIRE (PAR-Q)

The purpose of this questionnaire is to ensure that you are ready and safe to begin physical activity. Please read each question carefully and answer honestly.

1. Has your doctor ever said that you have a heart condition or that you should only do physical activity recommended by a doctor?

2. Do you feel pain in your chest when you do physical activity?

3. In the past month, have you had chest pain when you were not doing physical activity?

4. Do you lose your balance because of dizziness or do you ever lose consciousness?

5. Do you have a bone or joint problem that could be made worse by a change in your physical activity?

6. Is your doctor currently prescribing medication for your blood pressure or a heart condition?

7. Do you know of any other reason why you should not do physical activity?
    YES   NO
